In consideration of my acceptance to set up a donation page on this site regarding my fundraising event for the Pat Tillman Foundation, I hereby for myself, my heirs and administrators waive any and all rights and claims for damages I may have against the Pat Tillman Foundation and will hold them harmless for any and all injuries suffered in connection with said event or with my event fundraising page. Also, the Foundation is not responsible for the loss of personal items or any other form of aggravation in connection with said event or event fundraising page. I have been warned I must be in good health to participate in the event (if applicable). In filling out this form, I also give permission for free use of my name and picture in any broadcast and print media accounting the event. In addition, I agree not to include any content on my fundraising page, nor conduct a fundraising event that may be considered offensive in any way. I acknowledge that the Foundation is not officially endorsing me as an individual, or my event (or any activities associated with my event) and that they are not responsible for the content of my event fundraising page. I understand that the Pat Tillman Foundation has the right to delete my event fundraising page at any time without notice. In filling out this form, I acknowledge that I have read and fully understand my own liability and accept the restrictions.
